Active Duty Service Members must provide a Leave and Earnings Statement (LES) to prove their military status and Military Spouses must provide a military ID and a copy of their spouse’s Leave and Earnings Statement. Veterans and Military Retirees must provide a DD Form 214 Certificate of Discharge under General or Honorable Discharge conditions. Gold Star Families must provide one of three forms of identification – a DD Form 1300, a DD Form 3 or additional documents that show your Gold Star status.
